Physiotherapist-led weight management starts with a clinical assessment rather than a target. We look at how you move, where you carry pain, how your joints tolerate load, and what your current fitness will safely allow. Everything prescribed follows from that assessment, so two people who arrive at the same weight rarely leave with the same programme.
Nutrition is planned from that same assessment rather than bolted on afterwards. Our in-house dietitian is an Accredited Practising Dietitian and a member of the Singapore Nutrition and Dietetics Association. She works with you on eating patterns that fit your household and your schedule, rather than a meal plan handed over and left to you. Around that, we look at the habits that keep the plan standing, from sleep to how activity is spread across the week.
Progress is reviewed on function and health markers alongside body
composition, so you can see strength, walking tolerance, blood pressure and
waist measurement improving even in weeks when the scale is slow to move.
Diabetes-Vitae runs as two programmes. Start wherever you are today, and your physiotherapist will confirm the right fit at your first assessment.

Your readings are raised but you have not been diagnosed with diabetes. This is the stage where structured activity and dietary change do the most work, so the programme is built around reducing your risk of progression and rebuilding habits that hold.

You are living with a diagnosis and want activity that is safe, sustainable and fits your week. The focus moves to day to day control, protecting against complications, and building capacity at a pace your body accepts.
We combine clinical physiotherapy with lifestyle medicine to deliver care that lasts.

Structured activity and dietary change to reduce the risk of progression to type 2 diabetes.

Prescribed exercise therapy and habit coaching to support day to day glycaemic control.

Safe activity planning around your glucose monitoring, coordinated with your treating doctor.

Guided movement during and after pregnancy, with follow up as your metabolic health settles.

Screened exercise and foot aware programming for reduced sensation and balance changes.

Graded conditioning where blood glucose and heart health need to be managed together.
Diabetes-Vitae is designed for anyone managing raised blood glucose, pre diabetes or a diabetes diagnosis who wants clinically supervised activity and dietary support, rather than general advice to eat better and exercise more.
Prescribed and monitored exercise supports glycaemic control, cardiovascular health and everyday energy, and it is safer where neuropathy, joint pain or cardiovascular risk are already part of the picture. Structure is also what makes change hold beyond the first few weeks.
Diabetes-Vitae is physiotherapist led and grounded in lifestyle medicine
rather than exercise alone. Exercise therapy and nutrition are planned from
the same assessment, with our
in house dietitian, an
Accredited Practising Dietitian and member of the Singapore Nutrition and
Dietetics Association, working on the dietary side. We work alongside your
treating doctor rather than in place of them.
You will begin with a detailed consultation and clinical assessment, followed by a personalised exercise and dietary plan, health coaching between sessions, and regular reassessment of your activity and health markers as you progress.
Your Diabetes-Vitae programme draws on a combination of the following, tailored to your readings, your fitness and your stage:
Individually prescribed exercise therapy, structured to support glycaemic control
Dietary advice and meal pattern support from our in house dietician
Health coaching and behaviour change support for daily routines, sleep and activity
Exercise safety screening and progress tracking of your health markers using digital tools
Diabetes-Vitae is for people whose readings have started to matter. That might be an HbA1c that came back raised, a pre diabetes result from a health screening, or a diagnosis you have been living with for years and now want to manage more actively.
It is a particularly good fit if you have been told to exercise more but are not sure what is safe for you, if general gym advice does not account for neuropathy, joint pain or cardiovascular risk, or if you are on medication and want activity planned around it rather than despite it.
We also see people whose numbers look controlled on paper but who have lost stamina, balance or confidence along the way. If weight is your main concern and your readings are normal, our weight management programme is the better starting point. Where both are in play, diabetes leads, and the weight side is managed within this programme, so that medication, diet and activity are reviewed as one plan.
Yes. Prescribed and supervised exercise is an established part of diabetes care, supporting glycaemic control, cardiovascular health and everyday energy. A physiotherapist screens for complications first, then builds a programme you can sustain safely. This complements the care of your treating doctor rather than replacing it.
The pre diabetes programme focuses on reducing your risk of progression, with the strongest emphasis on building new activity and eating habits. The diabetes programme focuses on day to day control, protecting against complications, and adapting activity around your readings and any symptoms.
A referral is not always required, though it helps us coordinate with your treating doctor, particularly if you are on medication or have a diagnosed complication. Our team can advise on this at booking, and we will always work alongside your existing medical care.
Dietary support is part of the programme, delivered by our in house dietician and planned from the same assessment as your exercise therapy. The balance between the two depends on your goals and your stage, and is agreed with you at the start.
Both need screening before you begin, which is precisely why the programme is clinically supervised. Where sensation, balance or readings are a concern, your physiotherapist adapts the type, intensity and footwear guidance accordingly, and coordinates with your doctor where needed.
Many people notice improvements in energy, stamina and daily routine within the first several weeks, with health markers reviewed at agreed intervals alongside your doctor. Results vary by individual, starting point and how consistently the programme fits into your week.
At PhysioVitae, we adopt a lifestyle medicine-based approach to physiotherapy, integrating evidence-based strategies for primary prevention (promoting wellness to avert health issues), secondary prevention (early identification and intervention), and tertiary prevention (managing conditions to minimise complications and foster sustained health).
Our structured process is designed to address your needs holistically, combining clinical expertise with personalised lifestyle modifications.
We begin with a detailed discussion to understand your health history, symptoms, and goals. This involves collaboration with specialist physiotherapists and, where indicated, other healthcare professionals in their fields of expertise.
Physiotherapy experts conduct thorough evaluations, supported by data and advanced technology, to identify root causes and develop a personalised plan aligned with lifestyle medicine principles.
We implement a targeted therapy plan that may include manual techniques, a tailored exercise programme, and diet modifications, all reinforced by behaviour change strategies to manage the acute issues and support prevention.
Progress is tracked using health technology and with regular in-clinic follow-ups to ensure ongoing effectiveness and adaptation.
We will equip you with personalised exercises, our specially curated evidence-based programmes for chronic disease management, comprehensive lifestyle recommendations, and innovative digital tools. These resources are designed to empower you in maintaining long-term health and preventing future issues across primary, secondary, and tertiary levels.
